  1. Easy. It's a Moorcock novel, which means it'll mostly be about Moorcockian concepts even if it's ostensibly a Doctor Who book. I wouldn't be surprised if the Doctor's only half in it.

  2. Yes, every review I've read of it has said that it breaks with canon to such an extent that it's only really Doctor Who in name.

  3. Use Tippex to change every reference to 'the Doctor' to the read 'the Eternal Champion' (or 'Jerry Cornelius'). Then print a random John Blanche picture off and stick on over the front cover.
